Frodo's and Sam's departure...

This title is pretty vague, but here's the reason: the post is a:

*SPOILER!*

Alright, we all know that Frodo left Middle-earth and sailed to Tol Eressëa at the end of the LOTR.
My question is, did Sam ever get to sail there? - after all, he WAS a Ring-bearer.
And if he did, and this is the question closest to my heart, did Frodo and Sam meet again?

I think this is a great idea - two life-long friends meeting one last time beyond the borders of the world.

It also says in the Appendix that Sam left his Red Book to Goldilocks, and then went West. According to the tradition of the Gamgees, he went to the Havens. So I'd say that's further support.
